Mid-Kansas Senior Outreach is a program to identify and make contact with isolated seniors, in Sedgwick County, who are facing mental health risks and other challenges of aging that threaten their ability to live independently and safely.

A Mid-Kansas Senior Outreach Care Coordinator and/or Mental Health Therapist will then contact the senior, assess his/her needs, and offer services which may include…

  • Care Coordination
  • Medical Referrals
  • Nutrition Programs
  • Abuse Interventions
  • Companionship
  • Social Activities
  • In-Home Mental Health Therapy
  • and much more

These services are offered at no cost to the senior and, if you wish, you can remain anonymous when making a referral.

Warning Signs to Watch For

  • Mental: Inappropriate responses, confusion, disorientation
  • Physical: Difficulty walking, hearing or seeing, untreated injuries, chronic illness, substance abuse
  • Emotional: Recent loss of a spouse, withdrawn, depressed or anxious
  • Behavior: Changes in activity level, agitation, anger, fear
  • Social: Isolated, no mention of, or apparent contact with, family or friends
  • Appearance: Poor hygiene, soiled clothing or bedding, changes in weight
  • Home: House in poor repair, trash or debris in yard, neglected pets, hoarding
  • Financial: Confused about money matters, unusual cash withdrawals, new “best friend”

Gatekeepers
